Please complete this form to request a program on topics including sexual health, mental health and mindfulness, physical health and alcohol, tobacco, and other drugs. If you have any questions regarding your request, please email us at firstname.lastname@example.org
Please include any relevant information (time, location, preferred topic, etc.) and
submit your request at least 2 weeks in advance.
To be respectful of everyone’s time peers will arrive at least 10-15 minutes before the program and will begin on time. If students are not organized or ready to begin on time, the peers are instructed to leave and reschedule any programs for a future date.
Please be there and be ready to begin at the scheduled time.
